Accessing our recovery programs is as simple as sitting down with your Fraser Health mental health professional—physician (family doctor), psychiatrist, registered social worker, or mental health clinician—and filling out our referral form. Referral forms must be completely filled out and signed by both the mental health professional and client. There is no charge for our recovery services.
If the referral is appropriate, the staff will arrange an orientation for the client, including a tour of the clubhouse, a description of the rehabilitation programs, and an explanation of the expectations and guidelines.

Once the form has been filled out, the Fraser Health practitioner will submit it to the applicable service.
