Accessing our recovery programs is as simple as sitting down with your Mental Health Professional and filling out our referral form (See the Referral page) to print a copy of this form.

For our recovery services there is no charge.

Referral forms must be completely filled out and signed by both the Mental Health Professional and the client.

Referral forms are filled out in a collaborative manner with both prospective member and their helping professional.
Helping Professionals listed to fill out the referral forms are: Physician (family doctor), Psychiatrist, Psychiatric Social Worker, and Mental Health Case Managers.

If the referral is appropriate, the staff will arrange an orientation for the client. Orientation includes both a clubhouse tour and description of rehabilitation programs together with an explanation of the expectations (rules) and guidelines.
